Diabetes

What if my student has DIABETES? 

Game tiles spell out diabetes.A School Health Plan, medications, and supplies MUST BE IN PLACE at the school BEFORE YOUR STUDENT CAN ATTEND. Please get in touch with your School Nurse ASAP. Who is my School Nurse?

Your School Nurse will create a School Health Plan specific for your student based on information you provide. This plan outlines what action staff must take to support your student’s diabetes management. It is distributed to staff that work with your student (including substitutes). 

PARENTS/GUARDIANS MUST PROVIDE THE SCHOOL NURSE WITH:

  1. RELEVANT MEDICAL HISTORY: Please complete Diabetes Care Information Form.
  2. RELEASE OF INFORMATION: Allows the School Nurse to communicate directly with your student’s healthcare provider.
  3. MEDICATION & ORDERS: Ask your student’s endocrinologist to provide a set of orders from their clinic specific for your student. Sometimes this is called a “School Care Plan”. Please sign this to authorize the orders in the plan. 
  • Insulin 
  • Glucagon (if prescribed)
  • All Medications are Required:
    - To be in their original pharmacy labeled container
    - To match the Licensed Healthcare Provider’s order 
    - Not be expired

Be advised: Glucagon can only be administered by a School Nurse or Paramedic. If your student requires Glucagon when a School Nurse is not available, paramedics will be called. 

4.  SUPPLIESStaff assist student with blood sugar check

  • Low blood sugar (hypoglycemia) supplies (ie: glucose tablets, juice, carbohydrate/protein snack, etc.)
  • Insulin administration equipment (ie: pen, needles, alcohol wipes, pump, lancing device and lancets, test strips)
  • Glucometer with back-up batteries
  • Ketone strips

TAKE NOTE: 

  • Some families prefer that their student carry all diabetes management supplies and medications with them. If this is the case for your student, the School Nurse will verify all supplies are in place in order for your student to stay in school. 
  • Students that require nursing support to check blood sugar and/or administer insulin will receive necessary assistance and care. Families of elementary-age students with diabetes may be interested in Consolidated Health Services offered at Chase Lake Elementary.
