Life Threatening Health Conditions

Any student with a Life Threatening Condition needs to comply with the state law (RCW 28A.210.320) before they can attend school. These are legal requirements put in place to protect your student’s health and safety. A student must have:

  1. A Nursing Care Plan
  2. If needed, Health Care Provider Orders for Medication/Equipment, and/or Medication Authorization forms.
  3. If needed, medication and/or equipment as needed to support the health condition.

Medication forms may be found on the Frequently Used Forms page, you can also get hard copies from your school office.

These three essentials must be delivered to your student's school on or before Tuesday, August 21 in order to guarantee that your student will be able to start on the first day of school. If these elements are provided after the deadline date, first day attendance cannot be guaranteed.
